Making a Referral to Franklin Hospice

Can I Self Refer?

We do accept self-referrals from those who have been diagnosed with a life-limiting illness but we prefer that the referral comes via a health professional.  If you are considering a referral to Franklin Hospice, we suggest you talk to your primary health professional (in most cases, this will be your GP).  If you prefer, you can give us a call during working hours on 09-238 9376 and ask to speak to one of our Palliative Care Nurses - they'll be happy to discuss referrals with you.

For Health Professionals

We accept patient referral letters via fax 09-238 9323, mail: PO Box 118, Pukekohe, Auckland 2340 or via Healthlink - our Healthlink address is frankhos. You need JavaScript enabled to view it..  Franklin Hospice uses MedTech as our Patient Management System and if that is the system your practice uses, you may be able to refer directly from MedTech.
